Patient Request Forms

The downloadable patient request form (PDF) is completed offline by veterinary clinics and emailed directly to Dr. Khan. The information contained in those forms relates to animal patients and is handled in accordance with standard veterinary professional practices.

Text Messages

If you opt in to text communication on the request form, Dr. Khan may contact you via SMS for scheduling coordination. You can opt out at any time by replying STOP or by contacting us directly.
